Your Care Plan

Your ““Care Plan”” will be the key document in which the assessment and delivery of your care needs are recorded. The “Care Plan” will develop and change as your needs change. The “Care Plan” will be regularly reviewed as we observe your needs and abilities. This may take place on a daily basis in some cases. All “Care Plans” will be reviewed regularly and in detail on at least a weekly basis, to make sure we are delivering the care you need in the most effective way. We will actively seek your opinions and input when developing your “Care Plan”, and when making changes and amendments to it, so as to ensure that you and your family are fully satisfied with the care you receive from us.

Advocacy

Your right to determine your own choices will be respected, and the views and wishes of your family always taken into consideration. If you need skilled help or advice, you will always have the right to access someone who is experienced and qualified to speak on your behalf, particularly regarding finances or when decisions need to be negotiated and made regarding the services you need. If needed, we can help to introduce you to qualified advocacy services that will act on your behalf and in your best interest.
